Select Dental Management’s co-employment arrangement with ADP provides for ADP to administer payroll and benefits. Our organizational structure is split between non-clinical and clinical employees

How To Get Started
What We Need from Employees to Get Started:
- Complete the I-9 form by the deadline through ADP and provide acceptable documents to Practice Leader for employment authorization and identity
- W4 Form – Federal tax withholding completed through ADP
- State W4 Form – State tax withholding allowance certificate through ADP
- ADP Direct Deposit Form
- Employees email addresses (Used by ADP to communicate with Employee)
- After partnering with us, you will receive email notification from ADP TotalSource via email on “How to Start” the registration process.
Payroll Calendar
For all employees, SDM operates a bi-weekly payroll calendar:
Note – Doctors are paid monthly by the 15th of each month
SDM PTO, Holidays & Office Closure Days
- Paid Time Off (PTO) – Select offers a PTO program for all Full Time and Part Time employees
- Company Holidays – SDM offers up to 7 Company holidays for those that are scheduled to work on the respective days
Office Closure Days – Employees are entitled to receive 3 Office Closure Days for those that are scheduled to work on the respective days. Office Closure days are discretion of the Doctors for inclement weather or for other related needs

Who is Eligible?
Employees who work an average of 30 hours or more per week are eligible to participate in Select Dental Management’s healthcare plans. These individuals are additionally able to participate in the following benefits:

- Vision (Employee, E+S, E+C, E+F)

- Select Dental Management contributes $350 per month to healthcare premiums for those enrolled (Annual benefit is $4,200, which represents 70% of the healthcare costs for the most basic plan offered)
- Employees that elect to participate in Select’s healthcare will additionally receive employer-paid life insurance and long-term disability coverage
